A Death Save is required whenever a character's total Injury Penalty (I.e. the total of their Wound Penalty and the penalty caused by their current level of Shock) reaches -4 or worse. Death Saves are a Willpower Check with target of 7. On a fail, the character dies.
Generally speaking if a character passes a Death Save then they are not required to make another one unless they receive further injury or Shock (and only then if their total Injury Penalty is still at least -4). However if a character is knocked unconscious (I.e. fails a Stun Save) then any Stun Save they make to recover from unconsciousness also provokes a fresh Death Save if they have a -4 or worse Injury Penalty.
This concept is now depreciated, having been replaced by Trauma Checks.
